Patient Protection Plan Enrollment Process

Enrolling in the Patient Protection Plan is simple. Just follow these steps.

When we receive your specimen and requisition form containing your complete insurance information, an enrollment packet containing a letter explaining the Patient Protection Plan and two enrollment forms will be mailed to you. You will only receive the enrollment packet if the test requisition form indicates that you have a commercial insurance plan that is not contracted with Athena. So, make sure you and your doctor have filled out the form correctly and completely. It’s also helpful to include a front and back copy of your insurance card.

Once you receive the enrollment packet review it carefully and complete the two forms.

You can mail or fax the forms back to Athena with your payment. They must be received within 21 days of our receipt of your specimen. We accept check, money order, or credit card payments.
